Ambulatory patients in Hospital Selayang are catered by four Outpatient Pharmacy Units, based on the reviewing discipline/ clinic :
a) Level 3 (ext 3257) – Medical, Endocrine, Rheumatology, Hepatology, Geriatrics, Paediatrics, Ophthalmology, ENT, O&G, Staff clinic
b) Level 4 (I)(ext 2225) – Surgical, Nephrology, Psychiatry, Hepatobiliary, Anaesth-Pain, Palliative Care, Neurosurgical, ORL
c) Level 4 (II) (ext 4499) – Dermatology, Urology, Orthopedics
d) Emergency pharmacy (ext 2093) – Emergency, Daycare
Scope of service and activity includes, but not limited to:
a) Delivering medication-related services to ambulatory patients and health care providers
b) Monitoring patients’ medications and ensuring appropriate drug therapy received by patients:
o Reviewing prescriptions for proper drug and dosing
o Providing clear and complete instructions on drug therapy intake and storage
o Providing education to improve patients’ understanding and compliance to medications including TB patients
c) Counseling session on related medication regimens, specific administration methods, medication device handling.
d) Dispensing of home care medications over the counter.
e) Providing Value Added Pharmacy Services for medication refills.
Value Added Services ( VAS )
The services were introduced to facilitate patients and improve the quality of deliveries for medication refill collections. Types of services offered are; mail pharmacy (Ubat Melalui Pos), drive-thru (Farmasi Pandu Lalu), appointment card (Kad Temujanji), integrated drug dispensing system (SPUB).
Only refill prescriptions are allowed to request for these services and a new application is required for every prescription.

Ward Supply
The ward supply pharmacy provides services to the wards, clinics and other units in the hospital through the unit dose supply system, stock top-up system and imprest stock system. For the unit dose dispensing, each dose is packed individually and kept in patient bins. For discharged patients, bedside dispensing and counseling service is provided. Psychtropic and Dangerous Drugs are also supplied to the wards and units from the ward supply pharmacy.
CLINICAL PHARMACY SERVICES
Ward Pharmacy Services
Clinical pharmacists provide comprehensive patient-centered pharmaceutical care and participate in a wide range of clinical services and activities through interactions with patients, physicians and other health-care team members. These services include participation in specialist rounds, extensive patient-specific medication therapy monitoring, consultation with the medical staff regarding medication-related issues, and provision of drug information. Clinical areas served include: Intensive care, Renal and Liver Transplantation, Renal Pharmacy, Hepatology and Hepatobiliary, Cardiology, General Internal Medicine, General Surgery, Pediatric Care, Psychiatric, Palliative Pain Services, Urology, Ophtalmology, Orthopedic and Otorhinolaryingology. In addition, clinical pharmacists also serve as preceptors for Pharmacy clerkship and externship students.
Medication Therapy Adherence Clinics
Apart from serving the inpatient, the pharmacists at Selayang Hospital also serve clinic populations in the specialized areas of hepatology, renal and liver transplantation, diabetes and asthma. Patients are seen at the pharmacist-managed clinics known as Medication Therapy Adherence Clinic (MTAC) to ensure they are counseled on their medications, their side effects and educated with knowledge of their diseases. MTAC is an ambulatory care service which emphasizes on medication management. The purpose of this service is to focus on monitoring and management of the therapy whilst providing continuous individualized counseling. Medication aids to enhance compliance are also designed to suit the needs of each individual patient. Our pharmacists at this institution strive to provide patients with the best level of pharmaceutical care possible.
Total Parenteral Compounding And Cytotoxic Reconstitution
The Aseptic Unit provides a centralized area for compounding total parenteral nutrition bags, scheduled sterile IV medications and cytotoxic reconstitution. There is a level of competence and validation required before staff is permitted to work in the area and hence there is a significant training component for new staff. All products are prepared in a state-of-the-art clean room environment.
Pharmacists who work in these areas calculate the nutritional requirements for individual patients on TPN, calculates the dilution for cytotoxic medications, supervise the pharmacy technicians and perform the final checks for the compounding and / or reconstitution of these items.
Drug Information Services
This service provides information and advice on all aspects of drug therapy to healthcare professionals and the community. The service, which is free from commercial bias, can also be used by members of the general public. DIS is located at the Main Pharmacy Level 2.
Pharmacists at the Centre answer approximately 1500 enquiries each year about therapeutic drug use. Common enquiry types include questions about adverse effects and drug interactions as well as information about newly marketed drugs.
Pharmacists staffing the service have the clinical experience as well as access to resources such as the major medical databases (eg. Medline, the Micromedex and Lexicom) and an extensive in-house library of textbooks, journals and other printed reference material
Therapeutic Drug Monitoring Services
Drugs with narrow therapeutic index are monitored to avoid any possible toxic effects. These include amikacin, gentamicin, netilmicin, vancomycin, phenytoin, digoxin, valproic acid and benzodiazepine.
The Satellite pharmacy on Level 5 assists the Ward Supply Pharmacy by offering services to the O & G wards. The goal of the satellite pharmacy is to provide services in patient-care with the highest quality in mind. Among the services that are carried out by the Satellite Pharmacy include patient counseling, in-patient and discharge medication dispensing and replenishing floor-stock and after office hour medication stocks.
MANUFACTURING AND PRE-PACKAGING SERVICES
This is an important unit that specialized in formulation and manufacturing of creams, lotions, and mixtures in accordance with the Good Manufacturing Practice (GMP). This unit also pre-packs drugs for both inpatient and outpatient pharmacies.
PROCUREMENT AND SUPPLY SERVICES
Procurement and supplies of pharmaceuticals are carried out at the main drug store. It supplies drugs to the Main Pharmacy, Outpatient Pharmacy and Satellite Pharmacies. This store also supplies intravenous fluids and dialysis solutions directly to the wards.
RESEARCH AND EDUCATION
Research has increasingly become a key component activity in the recent years and is carried out in conjunction with clinical practice or other professional duties. Often, pharmacists work in collaboration with specialists and other pharmacists in the academic institutions and the national clinical research centres.
The Pharmacy Department is recognized by the Pharmacy Programme, Ministry of Health Malaysia as a training centre for pharmacists in nephrology, critical care and cytotoxic drug management and has trained pharmacists from other hospitals throughout the country.
